I once used an RSA USB device that randomly generated a four digit code, but since I wasn't able to see it, there was software available that put the code in a dialogue box on the screen. I was able to read it with the Window-Eyes mouse keys or use the copy button to capture the code and paste it into the authentication field.
Currently I am using Entrust which works very well. Although the Entrust ap for the Blackberry is not accessible when using the Blackberry screen reader, it works very nicely on iOS devices using VoiceOver.
