Frank: I imagine all of the comertial antispam appliances need a subscription of one kind or another, just like all comertial antivirus packages. You can build your own as suggested, but if you are still interested in looking at the comertial route, I've had good luck accessibilitywise with Sonicwall's E-mail security appliances. I can't vouch for how good the antispam is compared to others, but the UI is simple HTML pages. I'd also played with the Secure Computing, now McAfee, Ironmail and that worked relatively well but needed to be fussed with a little more to get the web pages to load properly. Finally, I'd look at outsourced solutions, such as Spamsoap and Microsoft's Exchange Hosted Services (different from their BPOS hosted Exchange.) Much less maintenance work, and most of them also archive and hold mail in case your Exchange server goes down. I've used Spamsoap.com, which is using the MXLogic engine, and the amount of spam blocked was excellent. It was pretty cheap too, I think the basic service was something like $2 per user a month.
