I used to run FreeSWITCH for this purpose (https://www.freeswitch.org/), and in fact I still do - but these days, it's hosted on a virtual server at linode.com rather than on a local machine, so it doesn't quite meet your criteria as presently configured. Apart from no longer having to deal with NAT issues in IPv4 networking, there's very little difference between the local and remote hosting scenarios.
