I've had success with Grml, which is also Debian-based. However, I haven't had a need for it in recent years, so I can't comment on its current state of accessibility. BRLTTY and Speakup are included by default. There are command line options for accessibility that you can specify during the boot process as documented on the GRML wiki, and once it boots, you are presented with a root shell - so it's all smooth from there.
