All screen readers do; System Access will install just as happily on a virtual machine as it does on a physical machine. So does JAWS. The GUI for VirtualBox is a bit difficult to use with a screen reader, but you can do everything from the command line. So, if you don't mind reading up on all of the command line commands and options, VirtualBox does seem to be quite usable. After a VM is set up and running, a screen reader (at least Window Eyes) works just fine inside the VM.
