Andrew: I haven't played with SQL Server 2008R2 or JAWS 12, but I see this all the time with other MMC apps and Window-Eyes, including SQL Management Studio 2008 non R2. I think it's a timing issue, AKA, once you hit down arrow, your focus is taking a little longer to change than the screen reader is expecting, so its assumed focus has changed and reads the selected item before things are ready. I've really not found a good fix for the problem, but I agree its very annoying. System Center Config Manager is horrible about this.
